Spain to Begin Supplying Medical Marijuana to Around 50 Hospitals This Week

Medical marijuana preparations are expected to begin arriving at approximately 50 hospitals across Spain this week, marking a major step in the country’s rollout of regulated marijuana treatments for patients with qualifying medical conditions.

Curaleaf, which became the first company to register standardized marijuana preparations with Spanish regulators, is preparing to supply approximately 50 public and private hospitals during the initial phase. Company executive Juan Pablo Martínez told Spanish newspaper El País that distribution will expand progressively as more hospitals begin requesting the products.

The treatments will be available to patients with four qualifying conditions: chronic pain that has not responded adequately to conventional treatments, spasticity associated with multiple sclerosis, severe treatment-resistant epilepsy, and nausea and vomiting caused by chemotherapy. Hospital specialists will determine eligibility and prescribe individualized preparations based on each patient’s medical needs and treatment history.

Under Spain’s Royal Decree 903/2025, approved in October 2025, medical marijuana preparations must be prescribed by hospital-based specialists and prepared and dispensed through hospital pharmacies. The system allows pharmacists to formulate treatments containing specific concentrations of tetrahydrocannabinol (THC) and cannabidiol (CBD), providing standardized dosing and medical supervision.

Curaleaf announced in July that the Spanish Agency for Medicines and Health Products (AEMPS) had registered two of its preparations, including one predominantly containing THC and another predominantly containing CBD. The products, designated CAN-1 and CAN-2, can be combined in different proportions by hospital pharmacies to produce individualized medications.

The agency has also published official formulation guidelines specifying how the oral preparations should be manufactured, prescribed and administered. The guidance recommends beginning treatment with the lowest possible dose and gradually adjusting it according to the patient’s response.

The initial hospital deliveries come approximately one year after Spain adopted its medical marijuana regulations. With the first supplies now expected to reach hospitals, the country is moving from establishing its regulatory framework to providing patients with access to standardized marijuana treatments through its healthcare system.
